2014 Medical Power of Attorney


this is a new form adopted by the Legislature for use beginning, January 1, 2014. This is the form for a medical power of attorney and related disclosure statements which are set forth in Sections 116.163 and 116.164 of the Texas Health and Safety Code.

2014 Statutory Durable Power of Attorney Basic


New form adopted by the Legislature for use beginning January 1, 2014: This is the form for a statutory durable power of attorney, as set forth in Section 752.051 of the Texas Estates Code.

Declaration of Guardian Adult


This is the declaration of guardian form set forth in Section 679 of the Texas Probate Code and Sections 1104.204-1104.205 of the Texas Estates Code, which permits an adult to designate the person he or she wants to be his or her guardian should the need later arise.

Declaration of Guardian For Children


This is the declaration of guardian form set forth in Section 677A of the Texas Probate Code and Sections 1104.153-1104.154 of the Texas Estates Code that permits a parent to designate a guardian for his or her children in the event of the parent's incapacity or death.

Directive_To_Physicians_witnesses


This is the directive to physicians and family or surrogates ("living will") form set forth in Section 166.033 of the Texas Health and Safety Code.
